We are asking parents to fill in their information online, including allergy information for Cadets. It is essential that you complete one of these forms for EACH of you cadets.  Failure to complete a food allergy form for your cadet(s) means they will not be permitted to attend FTX or other activities where food is being served.:

• Two parents can sign up on the same form or on separate forms.
  • If Parents are separated and both parents want to be registered each parent should fill out a separate form as Parent 1.
  • IMPORTANT NOTE:
    • If the Cadet DOES NOT have allergies both parents select NO (in the allergy section) and finish the form. .
    • If the Cadet DOES have allergies both parents should select YES BUT ONLY 1 PARENT SHOULD COMPLETE THE REMAINING QUESTION IN THE FOOD ALLERGY SECTION.
• MORE THAN ONE CADET? You need to fill out a separate food allergy section for each cadet!
  • For the 1st Cadet please fill in ALL sections (Parent 2 not required see above if parents separated).
  • For each additional cadet please follow these Instructions: Fill out ONLY the required fields for Parent 1, and leave the rest blank, select NO to Parent 2 and FILL IN THE FOOD ALLERGY SECTION FOR EACH CADET, sign it and submit it.
